New Patient Registration

registrationWe have an open list and welcome requests for registration from patients living in or moving to the practice area. Please speak to the Reception team for details of our current boundary area which includes all of Castleford, parts of Pontefract, Normanton  and Featherstone and  Methley. Please ask at reception where we can advise you whether you live in our catchment area.

You can register by bringing along your NHS medical card or completing a form available from reception. We will make a note on your records if you prefer to see a particular doctor or nurse.

If you are on medication, you will be advised to make an appointment with a doctor of your choice before you need a new prescription. We recommend that all new patients undertake a health check with a practice nurse. For patients who have any of the following problems, please make an appointment in one of our specialist clinics: - Diabetes, Coronary Heart Disease, Respiratory disease (Asthma or COPD) & Hypertension.

Please note that if you choose to complete your registration forms on line, you will still be required to call at the surgery to sign the forms in the presence of a witness and complete a new patient health questioniare. 


If you change your name, address or telephone number

If you change name, address or telephone number, please let our receptionists know by calling or writing to us, Please ensure that we know about other members of the household who may be registered with the Practice. If you move outside the practice area you will need to find a doctor in your new area.

 Don't forget to also inform any hospitals you may have appointments with.
